HIV and GERD -- What's Contraindicated?

Case Presentation

A 37-year-old man with HIV disease presents with a 1-week history of abdominal pain. His current medications include emtricitibine, tenofovir, atazanavir, ritonavir, and sertraline. A physical exam is unremarkable, with no guarding upon abdominal examination. His labs show the following:

CD4 count: 560 cells/µL
Viral load: nondetectable
Hemoglobin: 13 g/dL
Hematocrit: 39%
White blood cell count: 7 x 10⁹/L
Platelet count: 168,000
Alanine aminotransferase: 24 mg/dL
Aspartate aminotransferase: 30 mg/dL
Total bilirubin: 3.0 mg/dL
